Clay County Medical Center Spotlighted for Improved Care Transitions Processes
(Aug. 30, 2024) – Clay County Medical Center is a 25-bed Critical Access Hospital in North-Central Kansas with five Rural Health Clinics serving Clay Center and the surrounding counties. CCMC, a Kansas Hospital Association member, is also a Compass Hospital Quality Improvement Contract participating hospital through KHA's partner, the Kansas Healthcare Collaborative. Through the HQIC, CCMC identified opportunities for improvement in their care transitions, particularly between the hospital inpatient setting and their CCMC RHC clinics. KHC recently spotlighted CCMC for its improvement in care transition processes.
